What is a Home Care Package?
The Home Care Package program was established by the Australian
Government to assist older Australians to live independently in their own home.
The program is designed to provide flexible, tailored support according to
a person’s health care needs. Funding is allocated after formal assessment
and can include services to help a person stay:
Flexible Package Management
There are two types of management for your HCP Funding, giving you more control.
Self-Managed
Self-Managed tier is ideal for people who want to control their Home Care Package themselves or with the support of a family member.
Self-Managed PLUS
Self-Managed PLUS is ideal for individuals who want extra help from a care coordinator throughout the year.
Direct Care Services
Our services are designed to help with everyday tasks whilst providing friendly companionship, all in the comfort of your own home promoting independence, choice and control over the use of your funding.
Domestic Cleaning
Regular domestic cleaning around the home, with equipment and consumables to be supplied by client
Routine Gardening Maintenance
Light gardening maintenance and clean up, includes tools of the trade and fuel usage. Does not include debris removal.
Personal Care AND Home Help
Help with everyday tasks such as, personal care and hygiene, medication prompting, companionship, changing bed linen, in
home laundry, dishwashing, meal preparation.
Social AND Community
Access
Help with transport for
shopping and general appointments, community-based activities, local sport and recreation.
